If you prefer to print this three page survey, you may want to use the PDF (Portable Document Format) document to print the survey. You will need Adobe Acrobat Reader (available free from http://www.adobe.com/support/downloads/main.html) to print the PDF version.

(2) Online submission with E-Mail

If you prefer to complete this three page survey online, you may use the Microsoft Word version. You will need Microsoft Word (or Star Office, free for educational use from http://www.sun.com/products-n-solutions/edu/solutions/staroffice.html). Download the survey and save it to your local computer. Complete the survey and return it via e-mail to edfacdevel @ ed.umuc.edu .
